  • Permanent Placement

 
  1. Permanent Placement is staffed by Social Workers who:
    1. Manage Long Term Foster Care cases by:
    2. continuing to seek relative placements and more permanent plans, often by  holding Family Decision Meetings,
    3. reassessing the progress of parents in the hope of reunification, 
    4. seeing the child regularly,
    5. supporting and arranging visitation with relatives and parents,
    6. working with the foster families,
    7. writing a comprehensive review of the children's progress every 6 months,
    8. arranging special services such as counseling, or assisting with Social Security Income (SSI) applications,
    9. assessing and applying for Special Needs payment to foster parents for problematic children, and,
    10. finding new placements if changes become necessary.
  2. Ask the Court to approve trial visits with a parent or parents who eventually complete the court-ordered Case Plan and demonstrate a willingness and ability to parent their children.

Independent Living Skills Program (ILSP)

Eligibility:  All 16 - 21 year old foster youth

Goal:  Acquire skills to achieve self-sufficiency prior to leaving foster care

Functions:  Educational, vocational and social

Perks:  A small cash stipend for attending

Skills taught:

  1. Self-Esteem,
  2. Employment Skills,
  3. Home/Money/Time Management,
  4. Breaking the Abuse Cycle,
  5. Health Management and Safety,
  6. Relationships,
  7. Housing, Transportation, Community Resources,
  8. Social Skills,
  9. Decision-making/Goal Setting,
  10. Career/Vocational Goals/Financial Aids

Youth plan many social events throughout the year.  Annual Social Functions:  Retreats and Awards Banquet.

Transitional Housing Placement Program (THPP)

"The real test of independent living!"

Age:  17 - 18

Housing:  2 youth per apartment

Income:  Foster Care money

Number:  up to 6 youths

Intense Supervision by:   1 Social Worker

Selection Process:  Interview Panel

Requirements:  Passing grades, school attendance, part time job, good attitude, maturity level considered.  Students selected hold jobs, budget and manage their money, and adhere to a strict set of house rules under the guidance of their T.H.P.P. Social Worker.

After Care Coordinator

A Social Worker IV Coordinates this program

Clients:  Foster youth 17 - 21 who are out of school or in alternative education

Services:  Job search, job training, housing

Funding:  Federal Grant 1999
